Understanding Long Peripheral IV Catheters

According to the 2024 INS Standards, a long peripheral intravenous catheter (long PIVC) is:

"Inserted in either superficial or deep peripheral veins, long PIVCs offer an option when a short PIVC is not long enough to adequately cannulate the available vein."

Practice Recommendations from 2024 INS Standards
  • Ultrasound Guidance: For vessels that are difficult to palpate or visualize with the naked eye, it is recommended to use ultrasound guidance when inserting a long PIVC.
  • Evaluating Vessel Depth: Ensure sufficient catheter length within the vein by evaluating the depth of the vessel. A study in an adult emergency department showed a significant reduction in PIVC failure when two-thirds of the catheter length was within the vein.
